Clinical Lead, Chronic Care

CAN Health works with hospitals and payers to close the gaps in chronic disease management — the months between clinic visits where most outcomes are actually determined. As Clinical Lead for Chronic Care you will bring deep clinical expertise to our product and partnerships, ensuring that what we build is safe, evidence-based, and meaningfully useful to the patients and clinicians we serve.

What you'll do.

Responsibilities

  • Define and maintain clinical protocols for chronic condition pathways (cardiovascular, diabetes, respiratory) within CareOS.
  • Partner with hospital and payer customers to co-design care programmes built on the CAN platform.
  • Review AI-generated content and model outputs for clinical accuracy, appropriate scope, and safety.
  • Contribute to regulatory and compliance workstreams, including documentation for HIPAA, FDA Digital Health, and payer credentialing.
  • Serve as a clinical voice in product planning, ensuring features are grounded in how care actually works.

What we're looking for

  • Clinical degree and active licensure (MD, DO, NP, PA, or equivalent) with experience in chronic disease management.
  • 3+ years of clinical practice plus at least 2 years in a health technology, digital health, or clinical operations role.
  • Familiarity with value-based care models, remote patient monitoring, and care coordination workflows.
  • Exceptional written communication — you can translate clinical nuance for engineering and business audiences.
  • Collaborative by nature: you listen before you prescribe.
